Why Your Knee Pain Keeps Coming Back (Even When You’re Doing Everything Right)

With the recent passing of Chuck Norris, I saw a post about the Total Gym—that sliding bench machine that used to be everywhere.


Late-night TV. Daytime TV.



Chuck Norris and Christie Brinkley made fitness look effortless.


Most people wrote it off as just another infomercial gimmick.



But it highlights something most people with knee pain miss:


The real problem isn’t the exercises you’re doing.


It’s not knowing what your knee can actually handle.



Why Knee Pain Feels So Inconsistent

If you have knee pain, this probably sounds familiar:


  • Some days your knee feels fine.
  • Other days it hurts going down stairs.
  • Sometimes it flares up a day or two after a workout.

This inconsistency creates frustration—and confusion.



You might wonder:

  • “Was that exercise helpful… or harmful?”
  • “Am I making progress… or making it worse?”

This is where most people get stuck.  



The Real Reason Your Knee Pain Keeps Coming Back

Most people assume they need to:

  • strengthen more
  • stretch more
  • rest more
  • or “push through it”

The issue isn’t a lack of effort.


The issue is a lack of clarity.



If I ask, “How much force can your knees tolerate right now?”


Most people don’t know.



And without that clarity, everything becomes guesswork — which is why quick fixes don’t solve knee pain long-term:


  • Do too much → your knee flares up
  • Do too little → nothing improves
  • Try something new → hope it works

This cycle is what keeps knee pain lingering for months—or even years.



What Your Knees Actually Need to Improve

Your knees don’t improve from random exercises.


They improve when you gradually increase what they can tolerate.

This is called Load Tolerance.


What Is Load Tolerance?

Load Tolerance is the amount of force your knee can handle:


  • during a movement
  • for a specific number of repetitions
  • without triggering pain

It’s the difference between:


  • “This feels fine”

and

  • “This makes my knee hurt”



The Missing Measurement: Squat Load Tolerance

One of the most useful ways to measure knee progress is something called:

Squat Load Tolerance

This refers to:


        The percentage of your body weight your knee can handle without pain during a         squat.


For example:


  • If your knee tolerates 20% of your body weight → limited capacity
  • If it improves to 40–50% → significantly stronger and more capable



This matters because many everyday activities require at least 50% of your body weight per leg, including:

  • sitting down on a couch
  • standing up from a chair
  • getting out of a car



Why You Can Be Improving… Without Feeling Better Yet

Here’s one of the most important concepts to understand:


        Your knee often improves before you feel meaningful improvement.


For example:


  • You may go from 18% → 43% load tolerance
  • But still feel discomfort with stairs or daily movements



Without measurement, this feels like failure.


But in reality:

your knee is getting stronger—it just hasn’t crossed the threshold yet.


This is where many people quit too early.  



Why Most Knee Rehab Approaches Fall Short

Most programs rely on:


  • generic strengthening exercises
  • stretching routines
  • “listen to your body” advice



But they don’t answer critical questions:

  • How much load is safe right now?
  • How much is too much?
  • How should it progress over time?
  • Why do some exercises feel good, but don’t solve the problem?

Without these answers:

progress becomes inconsistent—or stops altogether.



What Actually Works for Long-Term Knee Pain Relief

Knee improvement requires three things:


1. A Clear Starting Point

You need to know:

  • what your knee can tolerate today
  • where pain begins



2. Precise, Measurable Loading


Not random exercises—but controlled, repeatable loading.  



3. Progressive Overload (Done Correctly)

Gradually increasing load over time—without triggering flare-ups.

This is the foundation of improving your knee from the inside out:


  • first improving the internal joint environment
  • then increasing load tolerance
  • then returning to higher-demand activities



The Key Takeaway

Your knees don’t improve from:

  • guessing
  • ice or heat
  • random effort
  • or doing more of everything



They improve from:


The right amount of load, at the right time, measured and progressed over time. 



Once you understand what your knee can tolerate:


  • you stop guessing

Why does my knee pain come and go?

Knee pain often feels inconsistent because your knee’s load tolerance is right near its limit. On days when the demand placed on your knee exceeds what it can handle, pain appears. When the demand stays below that threshold, your knee may feel fine. Without knowing your exact tolerance, this can feel random. 


Why does my knee hurt a day or two after exercise?

Delayed knee pain after exercise is often the result of irritation inside the joint rather than immediate injury. When the knee experiences more load than it can tolerate, small amounts of irritation can build up over time—leading to discomfort hours or days later. This is why pain doesn’t always show up during the activity itself.


What is Load Tolerance in the knee?

Load Tolerance refers to how much force your knee can handle during a specific movement without pain. It includes how much weight, how many repetitions, and how long your knee can perform an activity comfortably. Improving load tolerance is key to reducing knee pain and returning to normal activities. 


How do I know how much my knee can tolerate?

Most people don’t know their knee’s exact tolerance because it isn’t commonly measured. One of the most useful ways to assess it is through controlled movements—such as a squat—where the amount of load can be adjusted and tracked over time. This provides a clear starting point and a way to measure progress.  


Why isn’t strengthening my muscles fixing my knee pain?

Muscle strength alone doesn’t always address the internal capacity of the knee joint. If the joint itself cannot tolerate load, strengthening surrounding muscles may not resolve pain. Improving the knee’s internal load tolerance is often necessary before higher-level strength training becomes effective. 


What activities require higher knee load tolerance?

Everyday activities require different levels of load tolerance.  

For example:

  • Sitting down and standing up: ~50% body weight per leg
  • Stairs and inclines: often more than 70%
  • Running or sports: significantly higher loads

If your knee hasn’t built up to these levels yet, discomfort is more likely.


Can knee pain improve even if I don’t feel better yet?

Yes. Your knee can become stronger and more tolerant to load before you feel noticeable symptom relief. This is why tracking measurable progress is important—without it, many people stop too soon, even though improvement is already happening.
